How about:
-One births twerps with special needs, the other spews words are nothing special.

I was trying to go for the flow of a traditional British riddle with that one, but it was hard to fit.

I'll rephrase it:

One's words need special attention, the other's twerps have special needs.

How about - -

-The rambling sentences from out of the upper orifice sometimes successfully end in a period; the lower orifice only sometimes has periods too.

James, these are amazing replacement punchlines! I should do this more often!
I'm not going to reveal the "real punchline" until at least one person other than yourself contributes a replacement punchline, tho.

My friend Mrs Rachel E_____ suggests something along the lines of:

-Only one of them actually resembles a Pitt bull with lipstick.

Or some variant, the likes of:

-One wears lipstick & the other resembles an ugly wrinkly breed of dog. Et cetera.

ummmmm let us see....

damn, james' are just too good.

Hockey, Lips, Special Needs, Shooting the Shit, One you Roe into, the other you Wade out of (does that make any sense?) Ummmmmm
I'm no good at this.

Ah, so my initial instinct, that it had something to do with birthing special needs babies...

I don't know, my favorite was Liam's: One you Roe into, the other you Wade out of. Nothing like a little nonsensical supreme court humor.
